About Waythrough

Waythrough was formed in 2024 following the merger of Humankind and Richmond Fellowship. Together, we’ve created one of the largest mental health and social support charities in England.

Every year, we support around 125,000 people through nearly 200 services – and it’s all made possible by our 3,500 brilliant staff and volunteers.

Make a real difference in your community

Are you passionate about helping others live safer, healthier, more independent lives? Join our team at Waythrough and support people facing challenges around mental health, substance use, housing or domestic abuse. This is more than just a job – it’s a chance to build meaningful relationships and create lasting change.

About the Role

You will manage your own caseload, providing person-centred support from initial assessment and engagement through to treatment and sustained recovery.

Working closely with service users, colleagues, families and partner agencies, you will help individuals set meaningful goals, reduce the harms associated with substance use and build healthier, more independent futures.

Key Responsibilities

  • Manage a caseload and develop, deliver and review individual recovery plans.
  • Complete triage, screening and comprehensive assessments.
  • Carry out risk assessments, safeguarding plans and ongoing case management.
  • Deliver one-to-one and group-based psychosocial interventions.
  • Support harm reduction activities, including drug screening and blood-borne virus (BBV) testing.
  • Work collaboratively with partner agencies to coordinate effective support.
  • Connect individuals with treatment services, community resources and peer support.
  • Maintain accurate case records and produce clear written and verbal reports.
  • Work as part of a multidisciplinary team to achieve positive outcomes for individuals and their families.

About You

We are looking for someone who is empathetic, organised and committed to supporting people to make lasting positive changes.

You will ideally have:

  • An NVQ Level 3 in Health and Social Care, or equivalent, or a willingness to work towards this.
  • Strong communication, organisational and IT skills.
  • A compassionate, non-judgemental and person-centred approach.
  • The ability to manage a caseload and work with a range of professionals.
  • An understanding of recovery-focused approaches and the challenges associated with drug and alcohol use.
  • The confidence to work independently and collaboratively within a team.
  • A commitment to safeguarding, confidentiality and promoting positive outcomes.

Experience in substance misuse services, recovery planning, harm reduction or delivering individual and group interventions would be advantageous.

However, we are keen to hear from candidates with transferable skills who have the right attitude and a genuine desire to help others.

Why Join Us?

This is an opportunity to join a dedicated team where you can make a real difference to people's lives, support meaningful recovery and help individuals build a brighter future.

If you are passionate about helping others and ready to take the next step in your career, we would love to hear from you.

Apply today and become part of a team making a positive difference across County Durham.
